Have a female pregnant sim eat 3 apples. You will then have a boy.
If your female pregnant eats 3 watermelons, you will have a girl.
If you eat 2-3 watermelons and watch and listen to nothing but kid's stuff, you will have female twins or triplets.
If you eat 2-3 apples and watch and listen to nothing but kids stuff, you will have boy twins or triplets.
