You need to be doing Day 1 co-op for this.
Go to the Arcade and change to the Pistol.
Then aim and zoim in on one of the game screens and you will see "Game Over" pop up on the screen.
Shoot that screen 15 times and then Dizzy will get a Secret Hat.
